はじめに

　胆囊癌の治療においては，現在のところ根治手術のみが胆囊癌の生存率を改善できる唯一の治療法である．1982年から2004年の間に当科で切除した胆囊癌は53例で，壁深達度別の内訳は深達度m：4例，mp：6例，ss：26例，se：10例，si：7例であった．手術術式は，胆囊摘出術から拡大肝右葉切除＋膵頭十二指腸切除術（PD）まで様々な術式が施行された（表1）．総合的根治度（final curability：fCur）別にはfCur A：25例，fCur B：14例，fCur C：14例であった．fCur別の5年生存率はfCur A：78.3％，fCur B：27.8％，fCur C：0％で，非治癒切除例では3年以上の長期生存例は認められなかった（図1）．

　本稿では，術前診断から術式選択，術後補助療法に関する現在の当科における治療方針について述べる．
