はじめに

　膵癌の診断・治療は，これまでの拡大手術に対する再評価やMDCT（multidetector row CT）などの新しい画像診断装置の進歩，新規抗癌剤であるgemcitabine（以下，GEM）の登場などによって次第に変化してきている．近年，各種疾患でガイドラインが整備されつつあり，EBM（evidence-based medicine）に基づいた診断・治療が求められる時代になってきた．一方，膵癌でのevidence levelの高い検討は少なく，わが国におけるガイドラインも現在作成中である．また，DPC（diagnosis procedure combination）の導入などによる診療報酬体系の見直しといった医療経済的な側面も考慮すべき時代となってきた．

　本稿では，現在われわれが施行している膵癌の診断・治療における，（1）術前診療，（2）手術，（3）術後治療・処置のプロトコールについて述べる．
