はじめに

　当院における乳癌治療の基本方針は，手術に関してはでき得る限りを尽くして乳房温存を行うことである．腋窩リンパ節に関してはセンチネルリンパ節生検を積極的に行い，腋窩郭清を省略するという縮小手術主体の治療である．当院における治療スケジュールは初診から手術まではつぎのとおりである．すなわち，初診時に画像診断，組織診断を組み，がんの告知を行う．がん告知後は，看護師により告知後ケアガイドブック（後述）に基づいて行われる．その後，乳房温存術可能症例については手術を行う．乳房温存術が不可能な症例については個々の症例によって術前ホルモン療法や術前化学療法を行う．その後，乳房温存術可能になった症例（大多数の症例であるが）に対しては乳房温存術を，腫瘤の縮小が認めらない症例に対しては乳房切除術を行う．もちろん乳房温存術施行例には，原則として全例にセンチネルリンパ節生検を行う．転移陰性例に対しては腋窩郭清を省略する（図1）．これらについて順次，詳細を述べる．
