はじめに

　原発性肺癌は日本人の悪性腫瘍による死亡原因の第1位を占める予後不良の疾患であるが，治療の点からは小細胞肺癌とそれ以外の非小細胞肺癌に分類される．小細胞肺癌は原発性肺癌の15％程度を占め，早期にリンパ節および遠隔転移をきたすこと，また放射線や化学療法に対する感受性がきわめて高いことから外科治療の対象になることは稀である．非小細胞肺癌は，小細胞肺癌以外の組織型の総称であり，早期に発見して完全切除することが治癒に導く最も有効な治療法である．したがって本稿では，主として手術の対象となる非小細胞肺癌の外科治療を中心とした治療について延べることにする．
