Ⅹ．肺癌治療のプロトコール

はじめに

　わが国では悪性腫瘍が1985年以降死因の第1位を占めており，厚生労働省の平成15年人口動態統計（確定数）の概況によれば，年間56,000人が気管支および肺の悪性新生物で死亡している．当院においても肺癌手術例は年々増加している．

　2000年にも癌治療のプロトコールとして同様の企画で特集が組まれているが，この5年間に大きく異なったのは，診断面では，（1）空間分解能に優れたヘリカルCT〔helical CT，あるいはスパイラルCT（spiral CT）〕の普及により，従来では発見されないような微小あるいは小型肺癌が発見されるようになったこと．当院でも，肺癌手術例の増加は主としてⅠ期肺癌の増加によるものである（図1，表1）．（2）Positoron emission tomography（PET）が普及し，ある程度の質的診断が可能となったこと，である．治療面では，（1）1990年代に出現した新規抗癌剤が臨床の場で広く用いられ，その評価が定まりつつあること，（2）分子標的薬のような従来の抗癌剤とは作用機序の異なる薬剤が出現したこと，などが挙げられる．

　そのうち外科の領域では，（1）当時は未だ明らかでなかった術後補助化学療法の有効性が明らかにされつつあること，（2）胸腔鏡がほとんどの症例で使用され，Ⅰ期肺癌では，施設により胸腔鏡下肺葉切除術（VATS lobectomy）が標準術式となったこと，などが挙げられる．そのほか，evidence based medicine（EBM）やクリニカルパスの普及，種々の癌について治療のガイドラインが設けられた^1）ことにより，従来多くは経験に基づいて行われていた治療，処置の再評価が行われ，施設間，医師間の治療方針の差異が徐々に少なくなっていることである．

　本稿では，現在の当院における肺癌治療について述べる．
