生体肝移植におけるドナー評価

要旨：生体肝移植におけるドナー評価は，ドナー候補の自発的な意思を前提として，（1）ドナー肝切除を安全に行うことが医学的に可能であること，（2）移植されるグラフトがレシピエントにおいて長期的に機能するであろうこと，などを確認するプロセスである．その重要性については生体肝移植医療の開始当初から強調されており，近年になって国内外からドナー評価とドナーの術後経過に関する報告が増加している．欧米に比べて脳死肝移植例数が限られているわが国では，生体肝移植以外の選択肢は限られている現状にある．このため，ドナー候補の「選択の自由」を確保することが重要であり，あわせてドナー術後合併症への対策と，十分に配慮されたインフォームド・コンセントが必要である．
