成人間生体肝移植における過小グラフト問題

要旨：成人間生体肝移植における過小グラフト，いわゆるsmall-for-size（SFS）graftの問題は，生体肝移植という手技の性質上避けられない問題である．近年，成人間生体肝移植症例の増加によって，過小グラフトにから引き起こされるいわゆるSFS graft syndromeの臨床像，発症メカニズム及び対策も明らかにされつつある．本稿では，移植後肝不全の重要な因子の1つであるSFS graftにおけるこれらの病態と対策について解説し，今後の課題を含めて解説した．
