門脈血栓症における門脈再建および拡大左葉グラフトにおける肝静脈再建の工夫

要旨：生体肝移植においては，inflowである門脈血流とoutflowである肝静脈血流の両方をバランスよく確保することが重要である．Inflowを障害する門脈血栓症は肝移植のリスクファクターの1つであり，適応禁忌の時代もあったが，現在では血栓除去や血管グラフトを用いた再建などの工夫によって成績も向上した．また，生体肝移植ではoutflow blockが起こりやすく，肝静脈の再建に工夫が必要である．われわれは拡大左葉グラフトにおいてグラフト肝静脈を形成し下大静脈に直接吻合することによってoutflow blockを防いでいる．脳死移植が進まないわが国においては生体肝移植が中心とならざるを得ないが，合併症症例の1例1例に対して綿密に計画し工夫することによって脳死移植に遜色ない成績が得られるようになった．
