生体肝移植における免疫抑制療法

要旨：近年の優れた免疫抑制剤の開発は肝移植成績の向上に大きく寄与してきた．生体部分肝移植における免疫抑制プロトコールはカルシニューリンインヒビターとステロイドの併用が中心で，脳死肝移植のものと変わりない．良好な移植成績の一方で，慢性拒絶反応の問題や非特異的免疫抑制に起因する感染症や悪性腫瘍の問題が重要視されている．肝移植医療のさらなる発展には，正常な生体防御能を保ちつつ移植抗原に対する免疫応答のみを抑制し得るプロトコールの確立が重要であろう．本稿では，生体部分肝移植における免疫抑制療法の現状を概説し，一部の施設で行われている積極的な免疫抑制剤の減量・離脱の試みを取り上げた．また，われわれの免疫監視下での免疫調節の試みと，さらには免疫寛容誘導法の確立へ向けて足掛かりとなり得る肝類洞内皮細胞の免疫調節機構に関する研究成果の概要を紹介した．
