肝癌に対する生体肝移植

要旨：1996年にMazzaferroらが提唱した，いわゆるミラノ基準が現在，世界的なgold standardとなっており，この基準を満たした肝癌は肝移植のよい適応となっている．わが国においても肝癌に対する生体肝移植は増加の一途をたどっており，保険適用となった2004年からは特に急増している．ミラノ基準内の症例の3年生存率は78.7％と良好であるが，一方で肝癌症例の半数以上はミラノ基準外の症例であり，なおかつミラノ基準を逸脱した症例の50％前後は再発を認めていない．このことから，肝癌に対する肝移植適応基準の上限について，わが国独自の適応基準が求められている．さらに，移植成績の向上によって移植後患者生存率が移植以外の治療法と比べて遜色ないものとなってきたことから，肝癌治療全体における肝移植の位置づけを明確にし，どの段階から移植を積極的に適応とすべきなのか，内科医や放射線科医と連携して肝癌の治療体系を決定すべきである．
