食道癌手術における喉頭温存および胃機能温存手術

要旨：食道癌手術における機能温存（quality of lifeの向上）に向けての取り組みを，発声機能温存の観点からは頸部食道癌に対する喉頭温存手術を，胃機能温存の観点からは迷走神経温存食道抜去術を中心に紹介する．われわれは，頸部食道癌に対して術前化学放射線療法を行い腫瘍の縮小をはかったのち，気管，下咽頭への浸潤が認められない症例に対して咽頭輪状筋切開・喉頭挙上術を付加し，下咽頭直下から頸部食道を全摘する喉頭温存術式を行い良好な成績を挙げている．一方，秋山が開発した迷走神経温存食道抜去術は，内視鏡下手術の発達した現在，良好な視野で迷走神経を確実に温存できる術式と思われ，広範な食道粘膜病変を有する症例に対しては，新たな機能温存手術として再び脚光を浴びることが期待される．
