要旨：膵頭部癌に対しては従来，2/3の胃切除を伴う膵頭十二指腸切除（pancreatoduodenectomy：PD）が広く行われてきた．臓器機能温存の考えの普及とともに幽門輪と胃を温存する幽門輪温存膵頭十二指腸切除（pylorus-preserving pancreatoduodenectomy：PPPD）を行う施設が多くなってきている．根治性と機能温存のバランスが重要である．根治性からは問題となる5，6番リンパ節転移は高頻度ではなく，PPPDでも郭清は可能なこと，さらに再発率，生存率ともにPDとPPPDで差はないとするものが多い．合併症の面ではPPPDは早期に胃排泄遅延が多いとする報告があり，長期にはPPPDが栄養状態や体重回復がよいとする報告がある．PD，PPPDを比較しまとめた．
