再発食道癌の外科治療

要旨：食道癌の予後は集学的治療の進歩により改善しているものの，治癒切除後の再発症例は少なくない．再発後の治療についても化学療法，放射線療法の組み合わせや新しい薬剤の開発により奏功例を経験することが多くなってきたとはいえ未だ満足できるものではない．多くの場合再発患者の全身状態は不良で，多臓器再発や複合性再発の様式を取るため，その治療方針の決定に難渋することが多い．筆者らの教室では適応症例は限られるものの，切除可能症例に対し積極的に外科的治療を行うことにより比較的良好な予後が得られている症例を経験している．再発を積極的治療可能な早期に発見するための術後フォローアップシステムとともに，その治療成績向上のため，基礎的研究も踏まえた新たな治療法の確立が必要である．
