食道癌放射線化学療法後の遺残再発症例に対する非外科的治療

要旨：食道癌に対する放射線化学療法（chemoradiotherapy：CRT）は外科切除術に匹敵する治療成績が得られる可能性があることが報告され，普及してきた．しかし，CRTは高い奏効率が得られる一方，局所の遺残再発率の高さが大きな課題となっている．遺残再発をきたした場合，病状により治療選択が必要になり，手術不能な局所進行例や遠隔転移例では抗癌剤治療が，局所のみの場合はサルベージ手術が行われている．しかし，サルベージ手術は術後合併症の多さの面から安全な治療法とは言えない．当院では局所のみの違残・再発例の場合内視鏡的なサルベージ治療に取り組んでおり，まだ短期的だが良好な治療成績を得ている．

　一方，治療不可能な遺残再発例では早期に食道狭窄を伴うため嚥下障害に対しては内視鏡的緩和治療を行い，患者のQOL向上に寄与している．
