急性肺塞栓症のIVRによる治療

要旨：急性肺塞栓症は急性期死亡率の高い疾患で，的確な診断と治療が不可欠となっている．抗凝固療法は最も古くから行われてきたが，生命の危機に瀕している致死性塊状型急性肺塞栓症には十分とは言えず，全身からの血栓溶解療法と外科的血栓摘除術が治療として選択されてきた．しかしながら，これらの治療法も決して満足な治療成績を収めてきたわけではない．カテーテルからの局所的血栓溶解療法や経皮的血栓摘除術，特殊なデバイスを用いた経皮的血栓破砕術，そしてステント治療はinterventional radiology（IVR）と総称される新しい治療法で，特に重症症例に対し行われ，比較的優れた成績が報告されてきている．本稿では，この新しい治療法について述べた．
