要旨：生体防御を高め，患者のoutcome改善を目的とする栄養法がimmunonutritionと言われ，注目されている．具体的には，早期経腸栄養法や免疫賦活作用があるとされる栄養素（アルギニン，ω-3系脂肪酸など）を豊富に含む製剤（immune enhancing enteral diet：IED）の投与が実践されている．待期手術患者では，IEDの術前・術後投与が術後感染性合併症発生率を約50％減少させるとする多くのエビデンスがあり，条件によっては術後早期投与あるいは術前投与のみでも有効性が確認されている．ICU患者でも外傷など主とする外科系疾患で同様の効果が報告されている．しかし，敗血症患者への投与では有益であるとの報告がある一方で有害との見解もあり，慎重に適応を決定する必要がある．
