要旨：正しい栄養療法の知識が普及し，漫然たる完全静脈栄養の施行は回避される傾向が強くなっている．上部消化管疾患の手術後に完全静脈栄養もしくは空腸瘻栄養が必須であるのは食道再建を要する手術後のみである．Major surgeryの術前に投与するimmunonutritionは，感染性合併症の発生頻度を有意に低下させることが判明した．Cost/benefitの点からもその有用性は高く評価されてよい．発癌や癌の増殖には脂質の代謝産物が深く関与している．今後，癌細胞内の環境を修飾し抗癌剤の効果を増強する目的で栄養療法が行われる可能性がある．
