要旨：慢性肝・膵機能障害の代表として，肝硬変と慢性膵炎の基本的病態，栄養管理について概説した．両者とも栄養障害が合併する頻度は高く，これらの患者に対する手術に際しては，術前に是正が可能なものには栄養療法を施行したほうがよい．肝硬変では機能的分類と病期を正確に診断し，ビタミンや微量元素も含めた栄養評価を行う．分岐鎖アミノ酸療法は代償性肝硬変にも有用性が認められ，今後，周術期栄養療法としての無作為化比較試験による評価が待たれるところである．慢性膵炎では病期と外分泌機能を評価しつつ，栄養障害に対して適切な栄養療法を選択する必要がある．
