要旨：消化器癌の分野における化学療法の進歩は目覚ましく，新規開発薬剤であるTS-1，CPT-11，Taxanes，L-OHPなどによる生存期間の延長とQOLの改善に寄与する化学療法の確立が期待されている．さらに，分子生物学的手法により，癌細胞の増殖因子やその受容体，アポトーシス関連分子，細胞周期制御蛋白を標的とする抗癌剤の開発も最新の癌化学療法として期待されている．それらと対照的に，従来から行われてきた放射線療法に関しても，近年，米国のMacdonaldらによって胃癌の術後化学放射線療法の有効性を示す信頼性の高いデータが報告され，消化器癌の化学放射線療法も再び注目されている．本稿では，新規抗癌剤の臨床応用と化学放射線療法の現状に関して最近の状況をまとめた．
