潰瘍性大腸炎の内科的治療―白血球系細胞除去療法の効果と問題点

要旨：潰瘍性大腸炎（UC）患者に対する新しい内科的非ステロイド治療である体外循環白血球系細胞除去療法（ELRT）について，現在主流であるフィルター式白血球除去療法（LCAP）とビーズ式顆粒球吸着療法（GCAP）を中心に外科治療との位置関係に重点をおいて解説する．ELRTの施行決定や使い分け，および再燃性の予測に大腸内視鏡検査による局所評価が重要であり，劇・重症例はもちろん，難治例のステロイド減量や待機手術導入の目的でも外科との密接な連携が必須である．ELRTは優れた臨床効果と高い安全性を両立した稀有な例であり，今後のさらなる開発とエビデンスの蓄積によって世界に発信できる新しい治療法としての確立が期待される．
