肝下部下大静脈血行遮断

要旨：肝切離面からの出血は，動脈・門脈のinflow system，肝静脈のoutflow systemの2系統があり，動脈・門脈のinflow systemに対してはPringle法が確立されている．一方，肝静脈からの出血に対してはこれまでは全肝血流遮断が施行されてきたが，肝切離面からの出血量を減らすという意味では手技が煩雑で適当でない．当科の検討では，肝静脈のoutflow systemからの出血は肝静脈の逆流によるものであり，肝下部下大静脈を遮断することで中心静脈圧をコントロールし，肝静脈からの出血を減少させられるとの結論を得た．
