Total hepatic vascular exclusion（THVE）法による肝切除術

要旨：肝部下大静脈への癌の直接浸潤，あるいは肝静脈から下大静脈内に進展する腫瘍栓など肝部下大静脈から肝静脈根部の血管処理を要する手術では，肝部下大静脈および肝を一時的に全身血行から完全に遮断するtotal hepatic vascular exclusion（THVE）法は術中出血量を抑え，air embolismなどの合併症を防止し得る有用な方法であるとともに，このような難易度の高い外科切除を確実に施行するための必要不可欠な手技でもある．一方，最近のMRI，MD-CTなど画像診断法の発達により，術前に下大静脈への腫瘍の浸潤範囲あるいは腫瘍栓の先進部位も正確に診断することが可能となっており，その進展範囲に応じた適切なTHVE法を選択することにより，安全かつ根治的な手術が可能となる．本稿では，THVEを併用した肝切除術の具体的な手技とその要点について解説する．
