GISTの定義と診断

要旨：消化管間葉系腫瘍の大半は平滑筋腫ではなく，gastrointestinal stromal tumor（GIST）である．GISTはKIT受容体型チロシンキナーゼやCD34を発現するなどカハールの介在細胞（interstitial cell of Cajal：ICC）と類似点が多く，ICCを細胞起源としていると考えられる．GISTの発生機構にはKITをコードするc-kit遺伝子の機能獲得性突然変異によるKITの恒常的な活性化が関与しているが，一部のGISTではPDGF受容体α遺伝子の機能獲得性突然変異が関与することも明らかとなっている．すべてのGISTを基本的に悪性のポテンシャルを有した腫瘍として扱うことが推奨されており，GISTの的確な診断が治療方針決定に重要である．GISTの病理診断にはKITやCD34の免疫染色が必要で，術前診断のために超音波内視鏡下穿刺生検が有用な場合がある．
