要旨：従来，平滑筋由来の腫瘍と考えられていた粘膜下腫瘍の多くは，今日では消化管間質腫瘍（gastrointestinal stromal tumor：GIST）に分類される．本稿ではGISTのなかでも頻度の高い胃GISTを中心に，外科手術の適応と方法を欧米での動向を踏まえながら概説した．また，近年は腹腔鏡下手術も積極的に行われており，その方法と注意点についても述べた．GISTの生物学的特性をきちんと理解して治療にあたることが重要である．
