脳血管障害を有する患者の術前・術後管理

　要旨：現在，世界一長い平均寿命を誇るわが国はまた，極端な高齢化社会を迎えている．患者の高齢化に伴い，高血圧や動脈硬化に起因した脳血管障害（脳卒中）に罹患する割合も増え，脳卒中後に抜歯や悪性腫瘍，外傷などのために外科手術が必要となる症例も多くなった．このため，外科処置と脳卒中の再発のリスクを客観的に評価し，予期していくことがきわめて重要になっている．本稿では，虚血性・出血性脳疾患などの脳卒中，また脳卒中後てんかんなどの概要を述べる．さらに，それらを有する患者の術前精査とその病態の評価，脳卒中治療薬の調整，術後管理についても解説した．
