胆管損傷修復後の胆管狭窄

　要旨：術中胆管損傷は腹腔鏡下胆囊摘出術の際が最も頻度が高いとされるが，損傷した手術時（golden chance）に適切な処置がなされないと術後胆管狭窄をきたし，繰り返す胆管炎から重篤な経過をとることがある．胆管狭窄例のほとんどは黄疸，胆管炎を合併しているため，まず胆汁うっ滞解除のための胆管ドレナージが必要となる．狭窄部の治療としてIVRによる保存的療法と外科的療法が挙げられる．最近では，バルーン拡張やチューブステント留置などのIVR治療により比較的良好な成績が得られ，治療の主体となっている．しかしながら，強固な瘢痕のため十分な拡張が得られない，あるいは一時的に拡張が得られても再狭窄をきたす場合，さらにはメタリックステントが留置された症例などでは，保存的治療ではコントロールがつかず，外科的療法が必要となることが多い．外科的治療では胆道再建，おもに胆管空腸吻合が選択されるが，手術では狭窄部周辺の瘢痕組織を十分に摘除し，健常な胆管を確実に露出して挙上空腸と吻合することがポイントとされる．
