胆道手術のための臨床局所解剖

要旨：胆道手術において重要なことは，肝十二指腸間膜内および膵内の胆道系，動脈系，門脈系の走行，変異について熟知することである．当科ではDIC-CTを応用したmulti-cholangiography法，およびvirtual duodenography法を施行し，術前診断および術中ナビゲーションとして利用している．同法は比較的低侵襲で，脈管系，臓器，病変の相互関係を短時間・単一の検査で把握できるという利点があり，症例ごとの検討が可能である．胆道疾患の術前検査としては，生検などinterventionを除けば，ほかの検査は実質的に不必要なことが多く，今後の普及が期待される．
