腹腔鏡下胆囊摘出術後に診断された潜在胆囊癌に対する対処法の検討

はじめに

　腹腔鏡下胆囊摘出術（laparoscopic cholecystectomy：以下，LC）は胆石症や胆囊ポリープなどの胆囊の良性疾患に対する標準術式として普及している．しかし，LC後に胆囊癌と診断される，いわゆる潜在胆囊癌の合併を認め，その頻度は0.23～2.85％と報告されている^1）．これら潜在胆囊癌に対しては根治的再手術の適応や施行するべき術式の選択が問題となる．また，初回手術をLCで行ったために重大な結果を招いたと思わざるを得ない症例が存在するのも事実である．

　本稿では，当院で経験した潜在胆囊癌の9症例を呈示し，当院における胆囊癌の治療成績をもとに潜在胆囊癌に対する対処法を検討した．
