外来経過観察中に腫瘍の増大を認めたため切除を行い，2年間無再発の膵原発カルチノイド腫瘍の1例

はじめに

　カルチノイド腫瘍は神経内分泌腫瘍の分類において最も頻度の高い腫瘍であるが，膵原発のカルチノイド腫瘍は非常に稀であり，その頻度は11,343例中156例（1.4％）である^1）．膵カルチノイドにおいてカルチノイド症候群が認められるのは23.3％で，そのほかの症状は腹痛（40.4％），下痢（30.8％），腹部腫瘤（21.9％），肝腫大（15.1％）などである^1）．典型的な症状が現れることは稀で，膵カルチノイドは診断時において76％に遠隔転移が認められ，5年生存率は28.9％である^1,2）．早期診断の難しさが予後に悪影響していると考えられる．

　われわれは膵原発のカルチノイド腫瘍を経験したので，若干の文献的考察を加えて報告する．
