腹膜播種性転移の病態メカニズムと分子標的治療

要旨：腹膜播種性転移の病態メカニズムについて分子生物学的な観点から述べる．癌細胞の原発巣からの離脱過程には，MMP-1産生による間質分解亢進，細胞間接着に関与するE-cadherinやdesmoglein-2の発現低下が重要である．腹膜との接着過程には，癌細胞CD44Hと中皮細胞ヒアルロン酸との接着，および癌細胞α2β1-integrin，α3β1-integrinと腹膜マトリックスとの接着が関与している．腹膜への浸潤過程には，腹膜の線維芽細胞が影響する．すなわち腹膜線維芽細胞により癌細胞の浸潤は強く促進され，また線維芽細胞の産生するHGFにより中皮細胞の形態変化や剝離が起こり腹膜転移をきたしやすくなる．これらの機序に基づいた細胞接着阻害剤や線維芽細胞抑制剤などの分子標的治療薬が期待される．
