癌性胸膜炎の診断と治療（悪性胸膜中皮腫を含む）

要旨：肺癌では癌性胸膜炎を高頻度に合併する．癌性胸膜炎は，胸腔内に播種所見がまったくみられないが洗浄細胞診断で癌細胞が発見される段階のもの，開胸して初めて微少な胸水や播種病変を指摘し得る段階のもの，臨床的に悪性胸水が貯留した段階のものがある．大量の胸水を伴った症例に対する胸水コントロールは肺癌を扱う医師にとって大切な手技の1つである．これらに対する標準的治療法が確立されているわけではないが，現在までのエビデンスに基づき，それぞれの段階での診断と治療法を概説した．
