スキルス胃癌腹膜転移に対する温熱化学療法―安全かつ効果的なchemo hyperthermic peritoneal perfusion（CHPP）の手技

要旨：われわれが開発した開腹法CHPPの手技の変遷と最新の手法について詳述し，スキルス胃癌における予防的および治療的CHPPの効果につき検討した．スキルス胃癌H0P0郭清切除症例におけるCHPP非施行例では5年生存率が12.5％であるのに対して，CHPP群では50％と腹膜再発予防効果と延命効果を認めた．スキルス胃癌H0P1に対する治療的CHPPでは，2年生存率44％，5年生存率は11％であり，CHPP非施行，非郭清切除症例に比して有意に延命効果を示した．CHPPにおいて20分以上のthermal doseを得た症例では20分未満の症例より有意に延命が認められた．開腹法の開発によりCHPPは効果的かつ安全に行えたが，術後は重症熱傷に準じた集中治療を要した．スキルス胃癌で腹膜転移を認めても，そのほかの非治癒因子が切除可能な場合は，積極的な切除とCHPPにより延命が得られる症例がある．CHPP施行中に加温効果を定量化することで精度の高い治療が行える．
