胃癌腹膜播種の化学療法

要旨：腹膜播種には，腹水，腸管の狭窄，尿管の狭窄などの様々な症状がある．これらの症状に対して，以前は全身化学療法だけでは十分な効果が期待できなかった．しかし，S-1や塩酸イリノテカン，タキサン系などの新規抗癌剤による治療で，それらの症状が改善する症例が認められるようになってきた．一方，腹腔内に直接抗癌剤を投与する局所療法はdose intensityが高いと期待され，様々な治療法の工夫が行われてきた．しかし，臨床試験で明らかな延命効果を示したものはほとんどない．ただし，化学療法の本来の目的は延命もしくは症状の改善であり，局所療法によって症状の改善効果が期待できるのであれば，延命効果が明らかでなくとも行う意義はあると考えられる．
