【1881年1月29日（1）―この日の胃切除の意味を探る手掛かり―】

　1881年1月29日が近代外科，特に腹部外科の発展の中で特別な日であることは言うまでもありません．この日WienのAllgemaine Krankenhausでは，Billrothが胃癌に対する胃切除（当時の用語では幽門癌に対する幽門切除）を成功裡に成し遂げ，その後，この患者の経過を4か月にわたって観察し得たのです．

　これは，一般には胃癌に対する外科手術の成功第1例として語り継がれていますが，当時の諸状況のなかに身を置いて考えてみますと，開腹して腹腔内の病巣を取り除くという外科手技を可能にしたこと，また消化管，特に上部消化管の連続性を離断しこれを再建することの可能性を実際に具現したということに第一義的な意味が置かれるべきものと思われます．つまり，1881年1月29日のBillrothにあっては，胃癌の手術という意識よりは，胃の切除手術という意味をより強く念頭に置いて手術を進めていたのではないかと考えるのです．
