膵―膵仮性囊胞，膵膿瘍

　要旨：近年の画像診断や内視鏡的治療技術の進歩によって，膵炎の合併症に対する非観血的治療の報告が増加している．膵仮性囊胞に対する非観血的ドレナージ術としては内視鏡的囊胞ドレナージ術（経胃・十二指腸的囊胞ドレナージ，内視鏡的経乳頭的囊胞ドレナージ）や超音波・CTガイド下経皮的囊胞ドレナージ術が選択されることがあり，症例によっては良好な成績を得ている．また，急性膵炎に合併した膵膿瘍に対しては経皮的囊胞ドレナージ術が第一選択の治療法となる可能性がある．膵仮性囊胞や膵膿瘍に対する非観血的ドレナージ術は外科的治療を選択する前に考慮されるべき治療法ではあるが，その適応や危険性について十分に検討することが重要である．
