急性胆囊炎に対するPTGBA,PTGBD,ENGBDの適応と限界

　要旨：急性胆囊炎に対するPTGBD，PTGBA，ENGBDの適応と限界について概説した．PTGBDは確実なドレナージ法であるが，患者のADLが制限される．また，出血傾向を有する症例や腹水，Chilaiditi症候群のような穿刺困難例などには適応とはならない．PTGBAは手技が簡便であるものの治療効果はPTGBDに劣る．また，PTGBDと同様に穿刺困難例には適応とはならない．一方，ENGBDでは穿刺困難例においても施行可能であるが，胆囊内挿管率とERCP後膵炎は大きな問題である．したがって，その適応は穿刺困難例でかつ手技成功率と偶発症発生に関して十分なインフォームド・コンセントが得られた症例に限定すべきである．急性胆囊炎の治療の基本は早期胆囊摘出術であるが，様々な理由により手術ができず，かつ保存的治療に抵抗性の症例では，これらの治療法の有用性と限界を十分に認識してドレナージを行う必要がある．
