　要旨：急性胆管炎と診断した場合，まずは抗菌薬投与などの保存的治療を施行したのち，治療が奏効しないときや，血圧低下，意識障害を伴う重症例では緊急胆道減圧が必要となる．最近は内視鏡の技術が進歩し，中下部胆管閉塞（胆石，腫瘍）ではまずendoscopic retrograde biliary drainage（ERBD）が最も非侵襲的であり第1選択となっている．ERBD不能例や肝門部狭窄による区域性胆管炎の症例はpercutaneous transhepatic biliary drainage（PTBD）が必要となる．一方，急性胆囊炎に対するpercutaneous transhepatic gallbladder drainage（PTGBD）は広く普及しており，ハイリスク患者のみならず，本法で胆囊炎をいったん改善させたのち，あるいは全身状態を評価したのちに手術を行うという治療法を採用している施設も多い．急性胆道炎の治療は各施設において最も安全で最適な治療法を選択することが重要である．
