鎖肛に対する腹腔鏡下手術の適応と限界

要旨：近年，小児領域においても腹腔鏡下手術が普及してその適応は拡大しつつあり，高位，中間位鎖肛に対しても腹腔鏡下手術が行われるようになってきた．その手術のポイントは瘻孔の確実な切離と正確なプルスルー経路の作成にある．そのために，当科では術中に尿道鏡（腟鏡）と腹腔鏡用筋刺激装置を使用している．腹腔鏡補助下造肛術は従来法（後方矢状切開法）と比較して直腸肛門付近の神経筋損傷が小さいため，術後の良好な排便機能の獲得が期待されており，今後，標準術式になる可能性があると思われる．一方で遺残瘻孔という合併症の報告もあり，瘻孔の処理を骨盤深部で行わなければならない中間位鎖肛に対してはその適応に議論がある．瘻孔の正確な処理が行えるような安全性を考慮した術式の開発が今後の課題である．
