肛門機能不全の診断・治療のコツと実際

要旨：肛門機能不全は，患者にとって肉体的のみならず，精神的・社会的にも負担の大きい病態である．臨床の場では，特に便失禁を訴えて来院する例が増加している．便失禁の原因は，女性では出産時の肛門括約筋の損傷による場合が多く，病態の把握には理学的所見のほかに生理学的検査，経肛門的超音波検査，defecographyなどが用いられてきた．括約筋の損傷部位が明らかなときは括約筋修復術が行われ，特発性便失禁に対しては保存的治療が中心となるが，外科的治療も考慮される．また，外傷などの大きな損傷の場合は有茎薄筋移植が適応となる．便秘に対しても各病態に応じた治療が必要となる．
