直腸脱の診断・治療のコツと実際

要旨：直腸脱は高齢者の増加に伴い，今後わが国でも増加が予想される疾患である．直腸の脱出だけでなく便失禁を伴う症例もあり，症状とともに直腸肛門内圧検査，デフェコグラフィーなどにより肛門機能を検索することは術後の便失禁などの排便機能を予測するのに有用である．直腸脱に対する手術術式には会陰側アプローチと腹腔側アプローチがあり，原則として高齢者には侵襲の少ない前者，全身状態が良好な若年者には後者が勧められる．直腸脱は良性疾患であるため，患者の状態，術式の難易度，術後再発率，合併症，術後排便機能などを考慮に入れて個々の患者に適した術式を選択することが必要である．
