肛門外科

はじめに

　人間にかかわる疾患のうち，発病初期から自覚症状がはっきりしているものが太古から注目されてきたことは十分に推察される．狩猟や人間同士の争いから生じる皮膚や筋肉，骨の外傷などはその代表的なものであろう．肛門病変においても疼痛，出血，脱出，腫脹，硬結触知などは，現代人が苦痛や不安として感じるように太古の人間も忌み嫌い，悩んだものと思われる．肛門疾患治療の歴史は長い．

　残っている記録は限られてくるが，今から3500年ほど前（紀元前1500年頃）に作られたとされる「Ebers Papyrus」（エジプト出土）には薬物によって痔核を縮小させる保存療法が記されている（図1）．3000年前（紀元前1000年）の古代インドで始まったアーユルヴェーダを基に編纂され，紀元前400年頃に完成した医書には各種肛門疾患の治療法が詳細に記載されている．2500年ほど前（紀元前500年頃）の釈迦はその経文のなかに眼や歯の病気に対する記述をし，肛門疾患に対する療法が「佛説療痔病経」に記載されている．一方，2400年ほど前（紀元前400年）のギリシャのヒポクラテス（紀元前460～377年）は痔瘻の診断・治療の詳細な記録を残しており，痔瘻の硬結については「ヒポクラテスの結節」として切除すべきこととして説いている．

　このように紀元前から肛門病の歴史はあるが，治療法においては欧米，東洋，わが国など地域によって，あるいは宗教を背景にした考え方によって様々に行われてきたようである．本稿では，わが国の肛門外科の歴史を，現在に至る肛門病診療の歴史とそこに大きな影響を与えた人物と治療内容で紹介し，また，現在の診断治療と将来の展望について述べたいと思う．
