文献概要

　要旨：胃癌の治療はD2郭清＋胃切除術が長い間標準術式として定着していた．しかし，近年では早期胃癌に対する標準的治療としてEMRが行われるようになり，ESDの登場によってさらに内視鏡的治療の適応は拡大されている．一方，早期胃癌に対する外科的治療として腹腔鏡下胃切除術などの縮小手術が開発され，急速に普及してきている．また，進行胃癌に対しては化学療法の有用性が明らかにされており，多くのレジメンが開発されるとともに，現在，第Ⅲ相試験が進行中である．このように胃癌の治療法はますます多様化している．2001年に作成され，2004年に改定された「胃癌治療ガイドライン」には現時点で推奨される治療法とその適応が示されており，日常診療上の参考になると思われる．
