肝癌の再発診療に関する最新のデータ

　要旨：肝細胞癌の肝切除治療後の5年以内の再発率は70～90％と高率であり，再発に対する治療法の選択は重要である．わが国の全国集計によれば，肝細胞癌に対する初回治療では肝切除やRFAなどの局所療法が選択される割合が65％なのに対して，再発肝癌に対してはTACEが選択される割合が60％である．しかし，腫瘍条件と肝機能条件を満たせば再発巣に対する肝切除の成績は良好であり，再発の時期や様式を考慮したうえで，TACEのみならず局所療法も積極的に行っていくべきであると考えられる．
