血管外科と創傷治癒―重症虚血肢に対する最新の治療戦略

文献購入ページに移動要旨:血管外科領域における創傷治癒と言えば,閉塞性動脈硬化症(arteriosclerosis obliterans:以下,ASO)の重症虚血肢における虚血性潰瘍の治癒が重要である.食生活や生活様式の欧米化,ならびに高齢化によってASOが増加し,末梢動脈疾患(peripheral arterial disease:以下,PAD)患者の90%以上を占め,欧米と同様にPAD=ASOと考えてよい時代となった.PADに対する治療は運動療法や薬物による内科的治療と,血行再建術として血管内治療および外科的バイパス術があり,Fontaineの分類に沿って治療方針を決定するのが原則である.FontaineⅡ度で運動療法や薬物療法で症状改善がみられず,また血管内治療の適応がなく,手術を希望する症例や,Fontaine Ⅲ,Ⅳ度の耐術可能な重症虚血肢(CLI)が外科的バイパス術の適応である.一方,血行再建術の適応とならないCLI症例に対して,近年,新しい治療法として血管新生を誘導することによって血流改善を促す血管新生療法(therapeutic angiogenesis)が試みられている.本稿では重症虚血肢に対する診断,外科的バイパス術,血管内治療,血管新生療法について解説する.
