膵癌術前化学療法の意義

要旨：膵癌は診断時にすでに手術不能な高度進行癌が多く，非常に予後不良な疾患である．唯一手術のみが根治の可能性のある治療法だが，切除可能と思われるような症例でも顕在化しない微小転移巣を有することが多い．治療成績向上のためには手術や放射線治療などの局所治療に加えて全身治療である化学療法に期待が寄せられているが，いまだ膵癌の術前化学療法に関してはRCTによるエビデンスがなく，その有用性は明らかではない．術後のRCTで予後延長効果がみられたgemcitabineや新規薬剤であるTS-1^®などを基幹薬剤とした術前化学療法のRCTを早期に施行すべきである．
