術後創感染症の管理に関するエビデンス

はじめに

　術後合併症のなかで最も頻度の高い創感染症の対策がコスト削減の面からも重要視されている．米国CDC（Centers for Disease Control and Prevention：疾病管理予防センター）が1999年に発表した「手術部位感染防止ガイドライン」は手術部位感染を防止するために数多くの文献のエビデンスから導かれたガイドラインであり，わが国でもこのガイドラインに準じて外科臨床における感染制御の手法が経験的なものからエビデンスに基づいた対策へと大きく変革するに至った．

　創感染とは，手術創と手術操作が直接及ぶ部位（切開創，臓器，体腔）における感染であり，CDCで定義されている手術部位感染（surgical site infection；以下，SSI）がこれに該当する^1）．SSIの発生率は各サーベイランスの報告で特に消化器系手術に高いことが指摘されており^2），一般外科手術でも最も症例数の多い消化器外科手術後の感染創対策が急務と思われる．CDC勧告に基づき術前剃毛の廃止，鼻腔MRSA監視培養の実施，禁煙徹底，術前シャワー浴の奨励などの創感染予防対策は各施設で積極的に導入され，ランダム化比較試験（randomized controlled trial；以下，RCT）での結果報告も多いが，実際に術後感染が生じた場合（特に創離開にまで至った場合）の創管理や創処置法に関しての文献をエビデンスレベルに基づき考察した報告はない．術後創感染症対策として感染創の管理や処置に関する論文からエビデンスを解析し，治療方針を導きだすことが本研究の目的である．
