―呼吸器合併症を起こしやすいのは？―術式と呼吸器合併症

要旨：外科手術後に起こる呼吸器合併症は臓器によってその内容や重症度に違いがある．本稿では各臓器の術式と注意すべき合併症について概説した．呼吸器手術では肺合併症や胸腔内合併症，気管支瘻など様々な合併症が発生し得る．消化器手術では特に食道癌根治術で呼吸器合併症の発生率が高い．心臓手術では心肺負荷と密接な関係がある．内分泌外科では気管や胸壁の合併切除例に注意が必要である．移植外科は早期では主に感染や拒絶反応が，長期では主に免疫抑制剤による感染，腫瘍，薬物毒性が呼吸器合併症の原因となる．それぞれの臓器で呼吸器合併症を起こしやすい術式を理解することで，合併症を減らす工夫をすることが大切である．
