消化器手術における術後呼吸器合併症の予防対策

要旨：術後呼吸器合併症は手術死亡原因のなかでも頻度が高く，とりわけハイリスク例では周術期に予防対策を留意する必要がある．喫煙，高度肥満，慢性閉塞性肺疾患（COPD），活動性肺感染症，高齢，全身運動能力の低下，認知障害などの危険因子を有する例では，十分な問診を含め，正確な術前評価を行い，術前管理を尽くして手術に臨むべきである．また，丁寧な患者指導によって呼吸訓練や術前後の対策が重要になるとの理解を深め，機能訓練への前向きな姿勢を引き出すことが大切になる．高齢者社会の到来によってハイリスク手術が増えることが予想され，患者個々の呼吸障害の病態にチーム医療によるきめ細かな呼吸や全身運動のリハビリテーションを取り入れることが肝要と考える．
