―日本のスタンダード―食道癌に対する外来化学療法

要旨：食道癌に対する化学療法はこれまで入院治療が原則であったが，患者のニーズや医療状況の変化から外来化学療法の必然性が認識されており，さらに診療報酬の改定によって外来化学療法加算が算定可能となったことから，今後は外来治療へ徐々にシフトしていくものと考えられる．しかし，進行食道癌患者は必ずしも良好なPSとは言えず，外来通院治療の適用は十分な全身状態評価と社会的背景も考慮のうえ，決定しなければならない．また，地域・施設間格差のない医療環境の整備も必要である．食道癌化学療法の現在の標準治療である5-FU＋cisplatin療法をはじめ，多くの治療法が入院を要するため，外来化学療法が可能である治療法の開発が急務であるが，わが国では保険適応となっている抗癌剤は少なく，今後，海外で有効性が示唆されている新規抗がん剤の早期承認や，5-FUに代わる経口フッ化ピリミジン剤（TS-1，capecitabine）の導入が待たれる．
