外傷治療後に抗生剤投与は必要か

【素朴な疑問】

　一口に外傷の処置と言っても多種多様である．われわれ外科医はほとんどの外傷あるいは熱傷，膿瘍の処置などのあと，必ずと言ってよいほど経口の抗生剤を投与してきた．多くは抗生剤，鎮痛剤，胃薬がセットで，3日間投与が平均的である．

　はたして本当に，ほぼ外傷症例の全例に投与は必要なのであろうか．あるいは創の感染あるいは化膿防止に役に立っているのであろうか．
